Steve Biko died in detention in South Africa in 1977. Critical ethical issues are raised both by the conduct of the doctors responsible for Biko's care and by the subsequent response of the medical profession as a whole. Because those issues are relevant to all healthcare professionals everywhere, the Biko affair provides a useful case study in medical ethics. We discuss the case in this article, describing how we use it in our teaching.

Antipathy towards the possibility that God exists is a common attitude, which has recently been clearly expressed by Thomas Nagel. This attitude is presumably irrelevant to the question whether God does exist. But it raises two other interesting philosophical issues. First, to what extent does this attitude motivate irrational belief? And secondly, how should the attitude be evaluated? This paper investigates that latter issue. Is the hope that God does not exist a morally proper hope? I simplify this question by (...) interpreting the relevant attitude as an aversion towards the Christian picture of the world coupled with a preference for the naturalist atheistic picture. And comparing the content of those two world pictures, I argue that the attitude, though partly explicable, is morally unjustifiable, for clearly recognisable reasons. Regardless, then, of further concerns about the doxastic influence of this attitude, we ought to be ashamed of our antipathy to God, which reflects badly upon us. (shrink)

Requires signed TDF. ;An explanation of the possibility of empirical knowledge must establish how there could be an adequately non-accidental connection between our beliefs and the truth about the independent external world, and how this could be a connection in which we have good reason to believe. ;Foundationalist theories of justification offer the most promise of establishing this connection. But the best recent foundationalist theories fail. Even the allegedly basic judgements of (...) the content of one's sensory experience are not utterly secure. And neither are these judgements shown to provide adequate support for our perceptual judgements. Critical cognitivism manages only to describe, and not to establish the entitlement, of procedures we ordinarily take to be justificatory. Arguments for the thesis that judgements of the sensory content of our experiences provide criteria for our perceptual judgements turn out to be unsound, as do Michael Dummett's arguments for the thesis that we could not understand the sentences in which our perceptual judgements are couched unless we were able to recognize truth-conditions of these sentences whenever those conditions obtain. And arguments for the view that perceptual judgements provide the best explanation of our experiences fail even to establish the probability of our empirical judgements. ;Nor does epistemological naturalism disclose any reason to regard ourselves as entitled to dispense with the requirement that we establish a justificatory explanation of our beliefs. ;However, belief in a non-deceiving God does offer a good, and indeed a uniquely good, explanation of the non-accidental connection between our empirical judgements and the truth. Even if it cannot be demonstrated, the theistic explanation must be postulated if we are to possess the required foundational grounds for our natural faith in the reliability of those presuppositions by which we guide ourselves in forming the beliefs we are most inclined to call knowledge. SummaryIntense, sustained nursing lengthens inter-birth intervals and is causally linked with low natural fertility. However, in traditional settings, the effects of such nursing on fertility are difficult to disentangle from those of nutrition. Results from an prospective, direct observational study of reproductive function in well-nourished Amele women who nurse intensively and persistently but who also have high fertility are here presented. Endocrine measures show that ovarian activity resumes by median 11·0 months postpartum. Median duration of postpartum amenorrhoea is 11·3 months, (...) time to next conception is 19·0 months, and the inter-birth interval is 28·0 months. Average life time fertility is 6·8. High fertility in Amele women is due both to refractoriness of reproductive function to suckling stimuli, and to maintenance of equivalent age-specific fertility rates across the reproductive life span. In the present paper we give a precise definition of a hidden-variable theory for quantum mechanics, whereby we adopt the weakest possible definition of a hidden-variable theory, which is compatible with the assumption that the bounded observables of a quantum mechanical system are represented by the elements of the real part Ar of a W*-algebra A (of the most general type) and the states are represented by the “normal states” (in the mathematical sense) of A. We then go on to (...) show that an example put forward by Bell in 1966 satisfies our definition (Sec. 2). Finally we make use of Bell's famous theorem to show that for a sufficiently non-commutative W*-algebra A no hidden-variable theory in our sense exists (Theorem 3.3 and its corollaries). (shrink)

The orthodox presentation of quantum theory often includes statements on state preparation and measurements without mentioning how these processes can be achieved. The often quoted projection postulate is regarded by many as problematical. This paper presents a systematic framework for state preparation and measurement. Within the existing Hilbert space formulation of quantum mechanics for spinless particles we show that it is possible (1)to prepare an arbitrary state and (2)to reduce all quantum measurements to local position measurements in an asymptotic way (...) by unitary evolution processes without recourse to the projection postulate. A generalization to spin-1/2particles is also given. The theory presented provides a general mathematical and theoretical foundation for many practical schemes for state preparation and measurement. This Companion provides a fresh and comprehensive account of this outstanding work, which remains among the most frequently read works of Greek philosophy, indeed of Classical antiquity in general. The sixteen essays, by authors who represent various academic disciplines, bring a spectrum of interpretive approaches to bear in order to aid the understanding of a wide-ranging audience, from first-time readers of the Republic who require guidance, to more experienced readers who wish to explore contemporary currents in the work’s interpretation. The (...) three initial chapters address aspects of the work as a whole. They are followed by essays that match closely the sequence in which topics are presented in the ten books of the Republic. Since the Republic returns frequently to the same topics by different routes, so do the authors of this volume, who provide the readers with divergent yet complementary perspectives by which to appreciate the Republic’s principal concerns. Context: The need to reinvigorate medical confidentiality protections is recognised as an important objective in building patient trust necessary for successful health outcomes. Little is known about patient understanding and expectations from medical confidentiality.Objective: To identify and describe patient views of medical confidentiality and to assess provisionally the range of these views.Design: Qualitative study using indepth, open ended face-to-face interviews.Setting: Southeastern Pennsylvania and southern New Jersey, USA.Participants: A total of 85 women interviewed at two clinical sites and three community/research centres.Main (...) outcome measures: Subjects’ understanding of medical confidentiality, beliefs about the handling of confidential information and concerns influencing disclosure of information to doctors.Results: The subjects defined medical confidentiality as the expectation that something done or said would be kept “private” but differed on what information was confidential and the basis and methods for protecting information. Some considered all medical information as confidential and thought confidentiality protections functioned to limit its circulation to medical uses and reimbursement needs. Others defined only sensitive or potentially stigmatising information as confidential. Many of these also defined medical confidentiality as a strict limit prohibiting information release, although some noted that specific permission or urgent need could override this limit.Conclusions: Patients share a basic understanding of confidentiality as protection of information, but some might have expectations that are likely not met by current practice nor anticipated by doctors. Doctors should recognise that patients might have their own medical confidentiality models. They should address divergences from current practice and provide support to those who face emotional or practical obstacles to self-revelation. (shrink)

The world is not on track to achieve Agenda 2030—the approach chosen in 2015 by all UN member states to engage multiple stakeholders for the common goal of sustainable development. The creation of the 17 Sustainable Development Goals (SDGs) arguably offered a new take on sustainable development by adopting hybrid and principle-based governance approaches, where public, private, not for profit and knowledge-institutions were invited to engage around achieving common medium-term targets. Cross-sector partnerships and multi-stakeholder engagement for sustainability have consequently taken (...) shape. But the call for collaboration has also come with fundamental challenges to meaningful engagement strategies—when private enterprises try to establish elaborate multi-stakeholder configurations. How can the purpose of businesses be mitigated through multi-stakeholder principle-based partnerships to effectively serve the purpose of a common sustainability agenda? In selecting nine scholarly contributions, this special issue aims at advancing this discourse. To stimulate further progress in business studies, this introductory essay, furthermore, identifies three pathways for research on multi-stakeholder engagement processes in support of the Decade of Action along three coupling lines: multi-sector alignment (relational coupling), operational perception alignment (cognitive coupling) and goal and strategic alignment (material coupling). This paper forms an introduction to this issue, the contents of which arose directly or indirectly from a conference in May 2001 on Corruption of scientific integrity? — The commercialisation of academic science. The introduction, in recent decades, of business culture and values into universities and research institutions is incompatible with the openness which scientific and all academic pursuit traditionally require. It has given rise to a web of problems over intellectual property and conflict of interest which has even led (...) to corporate sponsors’ suppressing unfavourable results of clinical trials, to the detriment of patients’ health. Although there are those who see the norms of science developing to recognise the importance of instrumental science aiming at specific goals and of knowledge judged by its value in a context of application, none justifies the covert manipulation of results by vested interest. Public awareness of these problems is growing and creating a climate of opinion where they may be addressed. We suggest a way forward by the introduction of nationally and internationally-accepted guidelines for industrial collaboration which contain proper protections of the core purposes of universities and of the independence of their research. Some codes suggested for this purpose are discussed. We note that some universities are moving to adopt such codes of conduct, but argue the need for strong support from the government through its funding bodies. (shrink)

This full-length study of Plato's dialogue Phaedrus, now in paperback, is written in the belief that such concerted scrutiny of a single dialogue is an important part of the project of understanding Plato so far as possible 'from the inside' - of gaining a feel for the man's philosophy. The focus of this account is on how the resources both of persuasive myth and of formal argument, for all that Plato sets them in strong contrast, nevertheless complement and reinforce each (...) other in his philosophy. Not only is the dialogue in its formal structure a dovetail of myth and argument, but the philosophic life that it praises is also shaped by an acknowledgement of the limitations of argument and the importance of mythical understanding. By means of this correlation of form and content Plato invites his readers, through the very act of reading, to take a first step along the path of the philosophical life. (shrink)
